This website provides service information and links to online appointment booking. You can also call the office to ask about a service or arrange an appointment. There is no patient portal, account sign-in, or completed-form upload. Please do not submit medical records, symptoms, or insurance information through this website.

Online appointments and deposits
Online booking uses a short-lived browser cookie to associate your selected service and time with a secure Stripe checkout. Stripe collects the name, email address, and payment details needed for your deposit. Card details are entered with Stripe and are not stored by this website. After verified payment, the booking workflow records your name, email address, selected service, appointment time, and payment status in our scheduling and customer-management systems and alerts our office. We use this information to arrange your visit and apply your deposit. Please call the office for changes or questions.
Printable forms
The forms available on this website are download-only. This website does not receive, store, or transmit completed forms. Print them and bring them to your visit; do not email completed forms unless the office gives you a secure method to do so.
Website usage and booking analytics
We count page views, visits, navigation and phone clicks, video engagement, booking steps, Stripe checkout clicks, and confirmed deposits using anonymous usage identifiers. Analytics do not contain names, phone numbers, email addresses, symptoms, form contents, or payment-card details. Like most websites, the hosting, security, and aggregate analytics systems may process limited technical information—such as an IP address, browser type, request date and time, and pages requested—to operate, secure, troubleshoot, and understand general site use. We do not use this website to collect health information or make care or eligibility decisions about you.
Google Ads measurement helps us distinguish confirmed online bookings from advertising calls and clicks. Google may use advertising cookies and limited browser information to attribute a booking to an ad. We do not send patient names, contact details, selected services, appointment times, symptoms, payment-card details, or checkout URL parameters to Google, and we do not use this tracking to build health-based advertising audiences.
Consent-based AI voice and SMS follow-up
When a person affirmatively agrees to the communication-consent option on a FYZICAL-owned advertising lead form, we may use automated calls, an AI-generated or artificial voice, and text messages to respond to that person's request for information, discuss the requested service, and coordinate an office callback or appointment. Consent is not required to receive care or purchase services.
Automated calls and text messages may come from (856) 492-5787. We may make an initial automated call and no more than one safeguarded follow-up attempt for an inquiry, including a callback the person specifically requests. We send at most one proactive text-message opening for an inquiry and may respond conversationally after the person replies. Msg frequency varies. Msg & data rates may apply. Reply STOP to stop text messages or HELP for help. A person may also ask during a call not to receive additional automated calls. For support, call (856) 444-8405.

We do not send clinical advice, medical records, insurance information, payment information, or sensitive health information by SMS. Do not reply to a text with private medical information.
AI calls, recordings, and transcripts
Our automated voice assistant identifies itself as an AI virtual assistant. Calls may be recorded, transcribed, summarized, and reviewed to respond to the inquiry, coordinate follow-up, monitor quality and safety, and improve the communication workflow. Do not share symptoms, diagnoses, medical records, insurance information, payment-card details, or other sensitive health information during an automated call or by text.
We and our contracted communications, customer-management, hosting, and support providers may process the minimum contact details, consent evidence, call or message content, and operational results necessary to provide and safeguard this program. We retain consent evidence and communications records only for as long as reasonably necessary for these purposes and applicable legal requirements.
Mobile information
Mobile numbers, SMS opt-in information, and SMS consent are not shared with third parties or affiliates for their marketing or promotional purposes. We may use service providers solely to operate and support our communications, subject to appropriate safeguards.
